Application Procedure
Department application deadline: We recommend submitting your application as early as possible. Early applications are due by December 31st for the following Fall. Applications will be considered after this date as space is available and granted admission on a rolling basis.
Use the Graduate School website to apply on-line. http://www.sa.utah.edu/admiss/graduate.htm (print the application before submitting it on-line, so the copy may be included with the materials sent directly to the Department of Biomedical Informatics)
The following Application Materials must be sent directly to the Department of Biomedical Informatics:
NOTE: Please assemble your application materials into ONE ENVELOPE and mail to the address below.

- A copy of the on-line application submitted to the Graduate school.
- A 'Statement of Purpose' outlining your educational and/or working background, computer knowledge, and career interests (1-2 pages).
- Three letters of recommendation from academic people who know your academic capabilities in sealed and signed envelopes.
- Application fee ($45 US citizen or permanent resident; $65 international)
- Official transcript from every university attended in official sealed envelopes. The department will make a copy and forward the original transcripts to the University Admissions office.
- Official GRE or MCAT scores
- A small, current photograph
- Financial assistance form (if applicable). The University of Utah Office of Financial Aid and Scholarship website is www.sa.utah.edu/finance.
- International students must submit the "International Student Affidavit of Support," available at www.sa.utah.edu/admiss/appdownload . The financial support requirement is $24,910 plus $3,000 for each dependent.
- . International students must submit TOEFL scores. A TSE exam may be required and will be determined in a telephone interview.
